Dining Form

Please advise the Dining Steward no later than the Sunday preceding the meeting by completing the form below. If any member fails to meet this deadline but confirms dining after that day, he will be able to dine but may not be able to be seated where he requests, but will instead be seated where a place is available in the dining plan.
